How to remove sociable in certain page?

I have sociable plugin but I don't want it to be displayed in my contact page.

I'm using http://green-beast.com/blog/?page_id=136 as my contact form.

I also want to remove the word "Contact" in my contact form page as attached image.

Can someone please help?

Go to Edit>>Pages and edit that page. At the bottom of write page, you will see a option telling Sociable Disabled? Check that box, save the page and it will not show sociable on that page.

I'm done with the sociable but the contact title still can't remove. Can you please advise?
